Tufts Medical Center is seeking an experienced RN-Home Health-Case Manager to join our Healthcare & Medical Services team and support patients in the community through high-quality, patient-centered home-based care. As part of a renowned academic medical center in Boston and a key member of Tufts Medicine, you will help integrate clinical care, education, and research to improve outcomes for a diverse patient population. In this role, you will manage a panel of home health patients, providing comprehensive nursing assessment, individualized care planning, and ongoing case management in the patient’s home. You will conduct thorough physical and psychosocial assessments, develop and implement evidence-based care plans, and adjust interventions as the patient’s condition and goals evolve. Using sound clinical judgment, you will perform skilled nursing visits, including wound care, medication management, chronic disease management, and patient and caregiver education focused on safety, symptom control, and prevention of hospital readmissions. You will coordinate services across the continuum of care, collaborating closely with physicians, therapists, social workers, and other members of the interdisciplinary team to ensure timely communication, appropriate resource use, and seamless transitions from hospital or clinic to home. As a case manager, you will track clinical outcomes, monitor progress toward goals, and escalate changes in condition promptly, leveraging both in-person visits and telehealth tools where appropriate. Working within Tufts Medical Center’s collaborative and innovative culture, you will contribute to quality improvement initiatives, regulatory compliance, and best-practice development in home health. You will document all care and coordination activities accurately and promptly in the electronic health record, following CMS, Joint Commission, and organizational standards. You will help foster an environment of continuous learning by sharing knowledge with peers, participating in education and training, and staying current with home health regulations, evidence-based guidelines, and emerging models of community-based care. This position is field-based, requiring travel to patients’ homes within the service area. You will independently manage your daily schedule and route, balancing visit demands with thorough documentation and proactive outreach to patients and families. While working autonomously in the field, you will be fully supported by a knowledgeable interdisciplinary team and the resources of a major academic medical center, with opportunities to engage in cutting-edge practice, participate in research-related initiatives, and help shape community health programs. Successful candidates will bring strong clinical assessment skills, excellent communication and teaching abilities, and a deep commitment to patient-centered, culturally sensitive care. You should be comfortable working in diverse home environments, confident in prioritizing and organizing a dynamic caseload, and motivated to partner with patients, families, and community providers to help individuals safely remain at home and achieve their highest level of independence.
