Centers Health Care is seeking an experienced MDS Coordinator to join our Healthcare & Medical Services team and support our skilled nursing and long-term care residents through accurate and timely clinical assessment and documentation. As an MDS Coordinator, you will be responsible for overseeing the Resident Assessment Instrument (RAI) process, completing and coordinating all Minimum Data Set (MDS) assessments, and ensuring compliance with state and federal regulations. You will collaborate closely with nursing, therapy, social work, and other interdisciplinary team members to develop, implement, and update individualized care plans that reflect residents’ clinical status and goals. In this role, you will review clinical documentation, interview residents and families as appropriate, and enter accurate data into the MDS and related EHR systems to support appropriate reimbursement and high-quality care. You will monitor assessment schedules, track due dates, and ensure all required assessments are completed within mandated time frames. You will analyze clinical and quality data to identify trends, support quality improvement initiatives, and assist leadership in optimizing clinical outcomes and reimbursement integrity.
