Tufts Medical Center is seeking a compassionate and skilled Registered Nurse (RN) – Home Health Case Manager to join our Healthcare & Medical Services team. In this role, you will provide high‑quality, patient‑centered nursing care to adult and geriatric patients in their homes, coordinating all aspects of care to support optimal health outcomes and safe transitions across settings. As an RN Home Health Case Manager, you will complete comprehensive nursing assessments, develop and update individualized care plans, and deliver direct clinical care including medication administration, wound care, chronic disease management, patient and family education, and symptom management. You will collaborate closely with physicians, therapists, social workers, and community resources to ensure that care is coordinated, evidence‑based, and focused on the unique needs and goals of each patient. You will monitor patient progress, identify changes in condition, and initiate appropriate interventions, including timely communication with providers and families. Documentation will be completed accurately and promptly in the electronic health record in accordance with regulatory, accreditation, and Tufts Medical Center policies. You will play a key role in promoting patient safety, preventing hospital readmissions, and supporting patients in managing complex conditions at home. Drawing on the collaborative and innovative culture of Tufts Medical Center, you will participate in quality improvement initiatives, adopt best practices, and engage in ongoing education and professional development. You will precept or mentor newer staff as appropriate, contributing to a learning environment that values diversity, respect, and continuous improvement.
