Bishop Rehabilitation & Nursing Center is seeking a detail-oriented and clinically strong MDS Coordinator to join our Healthcare & Medical Services team. In this role, you will oversee and coordinate the full MDS 3.0 assessment and care planning process for our skilled nursing facility, ensuring accurate, timely assessments that reflect each resident’s clinical needs and support optimal reimbursement. Working closely with nursing, therapy, social work, and medical staff, you will gather and analyze clinical data, complete and submit MDS assessments, and help develop individualized care plans focused on the well-being, dignity, and goals of our elderly residents. As an MDS Coordinator, you will be responsible for managing assessment schedules, monitoring changes in resident status, and ensuring all documentation complies with state, federal, and payer regulations. You will educate and guide interdisciplinary team members on proper documentation practices, PDPM-related requirements, and quality measures, helping to drive high-quality patient outcomes and strong survey performance. In partnership with leadership, you will participate in quality improvement initiatives, monitor key clinical and reimbursement indicators, and help identify opportunities to enhance care, efficiency, and compliance. This position is ideal for an RN who is passionate about geriatrics and rehabilitation, enjoys working with data and regulations, and values a collaborative, team-based environment.
