Bishop Rehabilitation & Nursing Center is seeking an experienced MDS Coordinator to join our Healthcare & Medical Services team in our skilled nursing facility. As the MDS Coordinator, you will be responsible for coordinating and completing accurate and timely MDS 3.0 assessments and related documentation for our elderly residents, ensuring compliance with federal and state regulations. You will collaborate closely with nursing, therapy, social work, and other interdisciplinary team members to collect clinical data, develop individualized care plans, and support optimal reimbursement under Medicare, Medicaid, and other payor systems. This role involves reviewing clinical records, monitoring resident status changes, scheduling and tracking assessments, and entering precise information into electronic health record and MDS software systems. You will analyze assessment outcomes to help drive high-quality patient care, support quality measures, and identify opportunities for improvement in resident outcomes. Working within a culture that is committed to compassionate care, ethical treatment, and continuous improvement, you will participate in care conferences, provide education to staff on the MDS and RAI process, and help ensure that documentation accurately reflects residents’ needs and services provided. The MDS Coordinator will play a key role in promoting a family-focused environment and collaborative work setting that emphasizes personal growth, teamwork, and high-quality patient outcomes.
